Volunteer / Team Leader Forms
Please remember that ALL forms must be faxed into the office within 24 hours. If you are unable to fax or bring the reports in to the office in person please contact a staff member and give them the victims information so that a follow up call can be completed as soon as possible. Original reports must be received by the office as soon as possible.
Volunteer Court Support Form (Volunteers)
Volunteers must fill this form out when they are doing court support.
Victim Service Phone Log (Team Leaders)
Used for all telephone calls where a team in not required
Call for service form (Team Leaders)
Used for when a team is required only
Call Sheet (Volunteers)
Volunteers must fill this sheet out with their volunteer partner when they complete a on scene call
